ProActive Rehab Affiliate Scheme – Terms & Conditions
1. Introduction
These Terms & Conditions (“Agreement”) govern participation in the ProActive Rehab Affiliate Scheme (“Affiliate Program”). By joining, you agree to abide by the terms outlined below.
2. Eligibility
• You must be 18 years or older to participate.
• Affiliates must have an active audience (e.g., social media, website, or in-person network) relevant to rehabilitation, fitness, health, or wellness.
• ProActive Rehab reserves the right to accept or reject any application at its sole discretion.
3. Commission Structure
• Affiliates earn £10 per successful referral (e.g., new paying client).
• Commission is paid only when the referred client completes and pays for their first treatment and consultation in full.
• Payments are made via [Bank Transfer] on a [monthly] basis on the 1st of each month
4. Referral Tracking & Payouts
• Each affiliate is assigned a unique referral code or link to track sign-ups. The Client/Member must input your unique refferal code onto the new patient sign up forms to receive the 30% off their session price and your £10 payment.
• Self-referrals or fraudulent transactions will not be eligible for commission.
• ProActive Rehab reserves the right to withhold commission if fraudulent activity is suspected.
5. Promotion Guidelines
• Affiliates may promote ProActive Rehab via social media, websites, email, or word-of-mouth.
• False, misleading, or unethical advertising is strictly prohibited (e.g., misleading claims about services or benefits).
• Use of spam, unsolicited messaging, or paid ads using “ProActive Rehab” keywords is not allowed.
6. Termination
• Either party may terminate this agreement at any time, with or without cause.
• Upon termination, unpaid commissions will be forfeited if the affiliate has violated these terms.
• ProActive Rehab reserves the right to modify or discontinue the Affiliate Program at any time.
7. Limitation of Liability
• ProActive Rehab is not responsible for any indirect, incidental, or consequential damages resulting from participation in the program.
• Affiliates agree to indemnify ProActive Rehab from any claims related to their promotional activities.
8. Changes to Terms
ProActive Rehab may update these terms at any time. Affiliates will be notified of major changes via email or website updates.
